From prenatal nutrition and breast/chestfeeding insights to parenting education and emotional support, we’ve got you covered. Need assistance with childcare resources, baby supplies, or even managing the “baby blues”? Our comprehensive services are designed to provide the assistance you need, when you need it most.

Community Navigation is a FREE and universal service provided to all prenatal Guilford County residents through your OB/GYN.

Postnatal Navigation

We want to emphasize the significance of postnatal care in ensuring your child’s healthy development and overall well-being.

Our ultimate goal is to see every Guilford County family thrive and achieve milestones of success. To make this a reality, we have established a thoughtful and comprehensive approach to postnatal care that involves our dedicated team of Community Navigators.

Our Navigators will meet with you to provide personalize guidance and support at your child’s well-child visits at ages 12 months, 24 months, and 36 months. We believe that each family’s journey is unique, and these visits are an opportunity for us to ensure that you have the resources and information needed for your child’s optimal growth.

Our Navigators not only offer valuable insights during these visits but also serve as a bridge to our community partner programs. If there are specific needs or concerns, rest assured that we will make the necessary connections to ensure that all aspects of your child’s well-being are being addressed.

To further enhance our support, our Navigators will follow up with you 3 to 5 weeks later. This check-in allows us to ensure that things are progressing well and to assess if any additional resource referrals are needed. It’s a chance for you to update us on your child’s development and share any feedback on previous program referrals.
